Physician House Calls delivers personalized medical care to help older adults remain healthy, independent, and comfortable at home
According to AARP, nearly 90 percent of older adults want to remain in their homes as they age. In response to this growing community need, Jewish Home offers a continuum of services both on its campus and through its “Jewish Home At Home” programs, helping older adults maintain their independence and remain safely in their homes for as long as possible.
One service is Jewish Home’s Physician House Calls that provides in-home primary care by a team of medical professionals.
The practice is welcoming a new provider this month, Maria LaFaro, DNP, RN, ANP-BC. She is a board-certified Adult Nurse Practitioner with more than 20 years of experience caring for adults across a wide range of healthcare settings, including primary care, emergency medicine, critical care, and home-based care. She spent many years as a clinical faculty member at the University of Rochester School of Nursing, helping educate the next generation of nurses. She holds a Doctor of Nursing Practice degree from Chatham University and master’s degree from the University of Rochester School of Nursing.
Physician House Calls is a team of medical professionals who travel to the homes of those age 65 and older to provide quality medical care that includes primary care visits, urgent evaluations for acute issues, care coordination, medication management, and after-hours on-call support.
The coordination of care is a team approach involving specialist physicians, a registered nurse answering patients’ calls, and a social worker available as needed. Physician House Calls works closely with other medical specialists and home health care and nursing agencies.
“Physician House Calls is a unique service that offers the best possible personalized and compassionate care, enabling older adults to be happy and healthy in their own homes,” LaFaro said. “Our practice has literally kept hundreds of older adults in their homes and at area independent or assisted living communities since we provide medical care directly in their homes. I am excited to be a part of this very fulfilling and much-needed service to older adults in our community.”
With one-quarter of Monroe County’s population over the age of 60, the demand is growing dramatically for increased care and services for older adults. Jewish Home’s expanding ecosystem of care of “Jewish Home At Home” and community-based programs include licensed home care, outpatient PT & OT rehabilitation, companion care, Personal Medicine of Rochester, a variety of memory care support services, and TekHub technology support.
While Physician House Calls is not Medicaid-eligible, most other insurances are accepted.
