Frequently Asked Questions
Is this insurance?
No, Living Well Connections is a care coordination membership program with some wrap-around services to support aging in place. You will still need Medicare or an equivalent health care plan/insurance and will remain responsible for any medical or healthcare expenses.
What if I need more care coordination hours than my plan includes?
Additional hours are billed at a discounted rate. Premier and Signature plans include rollover hours for flexibility month-to-month.
Can I upgrade my plan?
Yes, individuals can upgrade at any time. They would have to sign a new agreement and pay the new monthly fee associated with the plan.
Is there a membership commitment?
There is a 3-month minimum commitment for continuity of care, then members may cancel anytime with a 30-day written notice.
What if I move outside of the service area?
Currently the service area is Monroe County. If you move, just provide the written 30-day notice for cancellation.
