Is this insurance?

No, Living Well Connections is a care coordination membership program with some wrap-around services to support aging in place. You will still need Medicare or equivalent and will remain responsible for any medical or healthcare expenses.

Additional hours are billed at a discounted rate of $100/ hour, and Premier and Signature plans include rollover hours for flexibility month-to-month.

Yes, individuals can upgrade at any time. They would have to sign a new agreement and pay the new monthly fee associated with the plan.

There is a 3-month minimum commitment for continuity of care, then members may cancel anytime with a 30-day written notice.

Currently the service area is Monroe County. If you move, just provide the written 30-day notice for cancellation.
