How to Enroll
Our friendly associates are here to help you enroll in the Physician House Calls program. It’s easy to get started.

Call (585) 244-5993 and talk with our enrollment specialist.

The enrollment specialist will email or mail you a new patient admissions packet to complete including a patient enrollment form, release of records from your current primary care provider, and a telehealth consent form.

You will also receive information regarding our privacy policy, administrative fee, enrolling in our patient portal, and additional information regarding the program.

Once all of the paperwork is complete, we’ll contact you to schedule a new patient appointment with one of our providers.

Our program includes:
- In-home primary care by a team of medical professionals
- In-home urgent visits for medical problems
- Coordination of care with specialist physicians and home health care services
- Team approach with social worker and psychiatric nurse practitioner available as needed
- Medication management
- Home bound, non-driving adults over age 65, residing in the zip codes we cover
- Our program Is not Medicaid-eligible, but we do accept most other insurances

Brian Heppard, MD
Dr. Brian Heppard is board-certified in Family Medicine, Geriatrics, and Hospice & Palliative Medicine, and has 30 years of experience. Dr. Heppard earned his medical degree from Rutgers New Jersey Medical School.
Maria LaFaro, DNP, RN, ANP-BC
Maria Lafaro, NP, is a board-certified adult medicine nurse practitioner. Maria earned her master’s and Doctor of Nursing Practice degree from the University of Rochester. LaFaro has served the Rochester community for over 20 years.
Suzanne M Gillespie, MD, RD, CMD, FPALTC, AGSF, FACP

Dr. Gillespie graduated from the University of Virginia School of Medicine and completed her residency training in internal medicine at the University of Rochester Medical Center. She is a graduate of the University of Rochester clinical fellowship in geriatric medicine based at Monroe Community Hospital and is currently a departmental fellow with research interests in fear of falling in long-term care enrollees and outpatient comprehensive geriatric assessment. Her clinical work is focused in long-term care, geriatric assessment, and palliative care. She is also a registered dietitian.

Who qualifies for Physician House Calls?
Our services are for individuals aged 65 and older who do not drive and meet certain medical criteria. Connect with our team to determine if you or your loved one qualifies for our services.
Do you provide services in my area?
Our practice license determines the areas we can serve. Contact our office to see if you reside in our service area
What insurances do you accept?
Most insurances are accepted. You can contact our office to confirm.
Is there a fee for this service?
There is an $80 monthly fee ($960 annually) to support administrative operations of having a medical provider care for you in your home. This fee is billed directly to patients and is not covered by insurance. Your medical care is billed separately in accordance with your insurance plan.
What types of care can you provide in my home?
Below are examples of visits you might normally have with your primary care doctor in an office setting that we can provide in your home:
- Routine primary care
- Annual/wellness visits
- Chronic condition management
- Medication reviews
- Evaluation of new symptoms
- Follow-up after hospitalization
- Blood pressure monitoring
- Minor illnesses/infections
Do I have to switch my primary care provider? Can I keep my current specialists?
You will need to transition your primary care doctor to Physician House Calls, but our team can coordinate ongoing care with your current specialists.
How do I change my primary care doctor to Physician House Calls?
Contact our office at (585) 244-5993 for a medical transfer form.
What happens if I have an urgent medical need?
A provider is on call 24/7 to answer urgent calls after hours and on weekends.
If I need hospital care, where would I be admitted?
In an emergency, call 911 and go to the nearest hospital. Patients can choose their preferred hospital.
